Session Three: Community Needs Assessment
The five-month CCBHC Foundations Data Learning Series will bring together CCBHC-E PDI grantees interested in learning about the foundational principles of how clinic-level data can be collected and analyzed to identify and address population health needs and disparities and drive clinical decision-making. This series will explore introductory opportunities to build infrastructure for the CCBHC-E grant data requirements (e.g., NOMS, Community Needs Assessment and Disparities Impact Statement) as well as feature case examples to foster peer-to-peer learning.
