CoE-IHS IA: Leveraging the CHI Framework in Integrated Care Practices

Join us on Thursday, April 17, 3-4 p.m. ET, as we kick off our action series on integrated care models. This session will discuss the Comprehensive Health Integration (CHI) Framework and other common integrated care approaches — the Collaborative Care Model, Care Coordination and Referral Practices, Behavioral Activation, Trauma-informed Motivational Interviewing and more — that enhance interprofessional collaboration in integrated health. Experts will explore how each tool complements the others, creating a holistic, patient-centered approach to care.

Through this panel discussion, attendees will learn the importance of a collaborative, multidisciplinary approach to improving outcomes and ensuring effective delivery of comprehensive care.
