Complex Care Certificate
The CCBHC-Expansion Grantee National Training and Technical Assistance Center (CCBHC E-NTTAC) has partnered with Camden Coalition to offer a valuable training opportunity to our CCBHC partners: The Complex Care Certificate.
This initiative empowers interdisciplinary care teams with the knowledge, skills and attitudes necessary to deliver truly whole-person, person-centered, equitable, data-driven, cross-sector, team-based care to individuals facing a combination of medical, behavioral and social barriers to good health. These teams can include community health workers, nurses, doctors, peers, social workers, case managers and other frontline complex care practitioners.
This program will help support you in assisting individuals with complex health and social needs improve their overall health and wellbeing. The Complex Care Certificate offers a unique opportunity for your team members to cultivate authentic healing relationships with patients, supporting them in achieving sustained behavioral changes aligned with their self-identified goals.
Register for the Complex Care Certificate:
- Go to courses.camdenhealth.org/redeem.
- Enter the code ccbhcnttac (all lowercase) and click “Validate.”
- Agree to the terms and conditions and click “Register.”
- Fill out a brief onboarding survey.
- Proceed to content.
