CCBHC Care Coordination Toolkit

Care coordination is the linchpin of the CCBHC model. There are many ways to engage in care coordination. CCBHCs can choose an approach that makes the most sense for the community served and reflects the community needs assessment. While many staff roles may have elements of care coordination, this toolkit offers guidance on aligning your care coordination approach with the SAMHSA CCBHC certification criteria and shares strategies that have been effective in meeting community needs and CCBHC requirements.

This toolkit is a resource for Certified Community Behavioral Health Clinics (CCBHCs), including SAMHSA CCBHC expansion (CCBHC-E) grantees, to use in planning, designing and implementing care coordination, as well as in continuous quality improvement (CQI). It highlights practical guidance, strategies for meeting CCBHC requirements, resources and tools that CCBHCs can use to coordinate care within their organizations and in their local communities.
