Enhancing Pathways to Care for Veterans
Certified Community Behavioral Health Clinics (CCBHCs) play an increasingly vital role in facilitating access to care, improving outcomes and addressing community needs. The CCBHC model allows organizations to take a holistic approach to health care for veterans by integrating care, mitigating bureaucratic barriers and emphasizing cultural competence.
This resource is for CCBHCs, including Substance Abuse and Mental Health Services Administration (SAMHSA) CCBHC Expansion grantees, to use as a guide for planning, designing and implementing services for veterans and members of the armed forces. It highlights the innovations of Valley-Wide Health Systems, a Federally Qualified Health Center in southeast Colorado, and demonstrates their approach to improving access to care for veterans and members of the armed forces.
